Diplomas, Degrees, and Certifications:
Please provide a copy of your high school diploma (or GED equivalency) along with any other degree or certification you currently hold or are working towards.

Physical/TB Test Required Upon Hire:

CPR/First Aid Certification:
PCAs need CPR/First Aid certification. Course is available at Mutual Aide, Greensburg. Contact 724-837-6134 then dial 0.

Mandated and Permissive Reporting Training Course
All staff and volunteers are required to take the online Mandated and Permissive Reporting Training Course offered by the University of Pittsburgh and then print out your certificate. The website for the training is www.reportabusepa.pitt.edu.

Virtus Training Course
In addition to the Mandated Reporter Training all staff and volunteers will need to complete the Virtus training and then print out your certificate. The website for the training is www.virtusonline.org/virtus. When completing the training, choose the Greensburg, PA (Diocese) option when selecting the Archdiocese/Diocese/Religious Organization.

Diocese of Greensburg Code of Pastoral Conduct

Act 168 of 2014
This form will need to be printed and completed. Section 1 will need to be completed by all applicants. Section 2, if applicable, will need to be completed by the applicants current employer or all former employers that were school entities and/or where the applicant had direct contact with children. Act 24 of 2011
This form will need to be printed and completed by all applicants.

Form I-9
This form needs to be completed by all applicants and will be completed upon your first day of employment.

Act 114
Complete the Act 114 Provisional Employment Statement documentation.

Employee Handbook:
Thoroughly read the Employee Handbook and submit a receipt accepting the terms and conditions of employment.

Attached are the electronic communication, anti-discrimination, and anti-harassment policies. Certify here that you have read them and understand them.

Safety Videos:
Watch three safety videos administered by the Human Resources Office discussing workplace harassment, fire safety, and blood-borne pathogens. The harassment and fire safety videos include a short quiz.

Allergy Ready Course and EpiPen Demonstration:
The Allergy Ready course can be found at www.allergyready.com. To access the course: click register and fill in your info, click care course, click launch module and complete all parts, and print out certificate of completion. Once you have completed the course, submit the certificate to the nurse. You will demonstrate the use of an EpiPen with the nurse.
